Cleanup and simplification of macroblock_load
Doesn't do anything now, but will be useful for many future changes.
Splitting out nei*our calculation will make MBAFF implementation easier.
Calculation of nei*our_frame value (actual nei*ouring MBs, ignoring slices) will be useful for some future patches.
貌似是对隔行扫描（如30i）处理的参数
现在要用的话，参数是--tff (top field first)或者--bff (bottom field first)
这类处理我觉得用avs或者ffdshow处理更好，而且是按需处理，不需要的话当然是0（off）
